Putting Together a Plan that Works
The VA Social Worker determines the scope of specific help each Veteran is able to receive. This initial "conversation about care" is an important one, and will establish the basic framework for the care plan.
Our suite of care includes a range of personal support for needs like bathing, dressing, making meals and taking medications. We can provide a helping hand with everyday activities like transportation and bill paying, or support for family caregivers who may need a break so they can work, travel, or run errands.
In addition to a suite of traditional services, we provide specialized care and support for Alzheimer's, Dementia, Parkinson's, and other complex diagnoses, for which our skilled caregivers are specifically trained.
